Learn about some of the options available with webforms on our website.

Some Key Features

  • Are highly customizable
  • Forms can be duplicated, making it easy to swap them out, or redesign them
  • Use conditional logic to create multiple paths within a webform. So if a user selects a certain branch, for example, specific options related to that branch appear, but options related to other branches stay hidden
  • Can be scheduled to open and close
  • Submissions can be sent to different email addresses depending on what the user selects
  • Can auto populate fields using custom URL's
  • Can break the form into multiple pages, which helps users get a sense for where they are in the process
  • Users can sign the form submission
  • Users can upload files

Process

Any new webform, or if an existing webform collects new types of patron information, needs to go through our privacy process. 

The basic steps are:

  1. Fill out the promotional request form 
  2. LMT will pass your request on to the Manager of Information Services
  3. Jennifer Wilson will guide you through the privacy process.
  4. LMT would build and test the resulting form
